The procedure of BABINSKY:

1. The patient is typically lying down in a relaxed position.
2. The examiner takes a pointed object, such as the handle of a reflex hammer or a tongue depressor, and gently strokes the lateral aspect (outer edge) of the sole of the foot, moving from the heel to the ball of the foot.
3. In a normal response, the toes will flex or curl downward. However, in the presence of the Babinski sign, the big toe dorsiflexes (extends upward) and the other toes fan out.

Interpretation:

• A normal response (toe flexion) is often seen in infants up to about 2 years old.
• In adults, an abnormal response (Babinski sign) suggests an upper motor neuron lesion, which could be due to conditions such as spinal cord injury, multiple sclerosis, or certain neurological disorders.
